Care Coordinator (Complex Care)

Cityworx are recruiting for an experienced Care Coordinator to join an established Complex Care business based in the Wakefield area.

As a Care Coordinator your role is to assist the Registered Manager in organising the day-to-day running of the service where you will be responsible for the management and scheduling of service user rotas as well as participating in the on-call rota

Requirements:

• NVQ Level 3 in Health & Social Care
• 1 years experience as a Care Coordinator
• Willing to participate in the on-call rota
• Full driving licence with use of own vehicle
• Experince of managing Complex Care packages

Main Duties: 

• Scheduling and managing service user rotas to meet staffing needs
• Assisting in the completion of audits
• Supporting in the development of care plans
• Managing and supportingstaff so they can perform their roles safely
• Working alongside the management team to ensure all quality standards and procedures are adhered to
• Participating and attending regular meetings
• Participating in the recruitment of support workers
• Managing and reviewing performance of Complex Care support workers through probation periods, management practices, supervision sessions, spot checks, training records and an annual appraisal
