Dunsville Medical Centre JOB TITLE: Care navigator RESPONSIBLE TO: Reception Team Leader HOURS OF EMPLOYMENT: CARE NAVIGATOR: 2 shift rotation Working 37.5 hours per week two shift rota - 7.20am 15.20 - 10.00pm 6.00pm. JOB SUMMARY: To provide administrative support to the practice team. To act as the first port of call for patients accessing practice services and ensure that they are dealt with in a friendly and efficient manner. Undertake chaperone duties as required once trained. Duties listed may vary from time to time as the needs of the practice change and through discussion with the Reception Team Leader and Practice Team. CARE NAVIGATOR DUTIES 7.20am-8am Unlock rooms - Clinical rooms, Stock room, Mangers Office Unlock front door and tv on Unlock reception cabinets. Check fridge in treatment room for samples. Fill printers with prescriptions and turn on PCs only rooms that will be used that day and reception and prescription printer. Check path links and assign to appropriate clinician- print any non-patients and put in non-patient folder for label to be requested. Download OOH Reports and NHS 111 Reports- Start attaching to patients records. Check practice email. Log onto phone 8am-onwards Pathlinks assign to requested GP and bowel assigned to Nurse. Answer calls from patients booking appointments and dealing with queries. Check practice email regularly. Deal with patients at front desk File sick notes, pathology forms any letters for patients in A-Z Box Ensure any prescriptions for the pharmacy are in envelopes. Accept samples from patients- ensure they have a form filled in and are logged on the samples list. Open internal and external post Scan documents onto records. Check document inbox. Tasks- In order of priority Bag samples ready for 11am collection Check Medsec Letters are on patients record from internal post. Action any change of address/Mobile Numbers Complete any District Nurse referrals. Assist any patients with stair lift. Complete any Online access requests. Check document inbox. 10am shift- When you come in you should start by doing the following jobs Register patients. Check Accurx texts. Answer calls from patients booking appointments and dealing with queries. Continue attaching documents. Check practice email regularly. Deal with patients at front desk File sick notes, pathology forms any letters for patients in A-Z Box Accept samples from patients- ensure they have a form filled in and are logged on the samples list. Open internal and external post, sort, and date stamp Scan documents onto records. Check document inbox. Tasks- In order of priority Check Medsec Letters are on patients record from internal post. Action any change of address/Mobile Numbers Complete any District Nurse referrals. Assist any patients with stair lift. 5.45pm 6.00pm Remove prescriptions from all printers including reception printers. Prescriptions for patients to be filed. Prescriptions for pharmacy in an envelope Any samples to be placed in the fridge. Print off appointment list for next day. Turn TV off. Update duty board for following day Ensure kitchen is tidy. Wipe down BP machine and childrens play table in waiting room. Lock front door 6pm Tasks Order of Priority Red Flag tasks Pathology Patient/Medication Queries Admin Appointment Blood test Appointments Mother & Baby Discharges Asthma Follow ups Recall/Asthma Reviews All others Adhoc Jobs Wednesday afternoon Charge laptops, lights and mobiles Print patient registration packs Bag and label records File patient records Get out any records that need sending to other practices. Digitalise patient records. Summarising records Refill Urine/Stool/Sputum Sample bottles Working after a bank holiday There will be a large quantity of OOH reports on this day which should be downloaded and processed as soon as possible and then following this medisec documents as usual. Sort documents as above and action those that need prompt attention as before but when scanning, scan a batch and then attach to ensure the workflow keeps moving.
