An exciting new opportunity to support unpaid carers working within a multidisciplinary team to complement community-based services.
To work with the IW Council Wellbeing & Access Hub to identify and support unpaid carers within the community for 5 days per week on a permanent contract.
As part of a small team, the post holder will provide front-line support to carers mainly by phone, with some face-to-face work. They'll offer assessment which could be in person or by the telephone. We are looking for an individual who has an understanding of the unpaid carer’s role and has experience of working with people in the community who require support.
The role also involves providing information, advice, and support to help carers reduce the negative impacts of their caring role and continue living their lives to the fullest.
