Case Management is a process devoted to the coordination, rehabilitation, care and support of people with complex clinical needs. It aims to facilitate independence and improve quality of life for individuals living within the community. This is usually conducted as part of the medico legal process.

As a result of increased demand for our case management services, we are looking for experienced HCPC registered Nurses or Occupational Therapists in the North West area to provide the high quality of service we are known for.

We are looking to recruit suitable candidates with experience of working with clients suffering from spinal cord injuries or traumatic and acquired brain injuries and children with complex needs. Applicants should have experience of working with clients in community settings. Strong polish speaking skills are desirable.

Various options for employment are available with options to work remotely as an employed member of staff or to work flexibly from home on a self-employed basis with full professional support. Part time hours available.

What you'll be doing:

  • To provide a comprehensive assessment of an individual's abilities and difficulties in relation to their desired goals in achieving independence in everyday activities as part of the client's claim for compensation.
  • To enable the client to achieve their optimum level of independence by the implementation and monitoring of a suitable Case Management Plan, utilising a goal setting process with documented outcome measures.
  • To proactively review implementation of this plan and take further steps.
  • To take account of expert evidence provided as part of the litigation process.
  • To enable our clients to make progress on their goals.
  • To take account of all current and relevant legislation.
  • To carry out a comprehensive assessment of need within Company timescales taking into account all relevant documentation. To discuss options for meeting these needs with the client, informal/formal carers and other relevant professionals.
  • To identify goals for intervention which are agreed with the client/deputy, document all outcomes in accordance with the Tania Brown Ltd case management processes and prepare monthly summaries detailing outcomes of intervention and new goals agreed.
  • To identify appropriate assessment tools, obtain a baseline assessment of your client and monitor outcomes of intervention.
  • To participate in the recruitment, training and supervision of support staff, this includes monthly supervision to all staff working with your client in order to ensure the appropriateness of treatment objectives, management of any personnel or employment issues and to ensure the provision of high quality documentation.
  • To formulate, document and implement Case Management Plan as discussed and agreed with client and/or family/legal representative within Company timescale.
  • To prepare review reports within Company timescales, but no less than every 12 weeks unless otherwise agreed with your line manager
  • To identify, arrange and co-ordinate assessments/treatment intervention with other relevant professionals through either statutory or private provision.
  • To monitor the effectiveness of such treatment to ensure a cost effective service for your intervention by the use of multi-disciplinary team meetings, client progress/outcome reports and verbal communication.
  • To identify care/support needs and enable the client to meet those needs via statutory or private provision including recruitment of independently employed staff.
  • To arrange, co-ordinate and attend at client multi-disciplinary team meetings as required but no less than every 12 weeks unless otherwise agreed with your line manager.
  • To accompany the client when attending for appointments etc, if required.
  • To represent Tania Brown Ltd as a witness of fact at case conference meetings as per the litigation process and in court proceedings, if required.
  • To be responsible for documenting all intervention relevant to the Case Management process as per Tania Brown Ltd operational procedures and professional standards.
  • To prioritise own workload and manage time effectively ensuring a minimum of chargeable time per day/period as agreed by Tania Brown Ltd.
  • To participate in the monitoring, development and operation of Tania Brown Ltd to ensure that standards of assessment and working practices are current and valid and to maintain high professional standards within the relevant code of professional conduct.
  • To assist in the development, supervision and training of assistant Case Managers or others within the team to provide a multi-skilled workforce, to include peer supervision as part of the case management process.
  • To attend/participate in monthly team meetings.
  • To manage own CPD, participate in the identification of personal training needs and to attend any courses required as part of your PDP identified in the appraisal process as detailed in the employee handbook.
  • To participate in business development through attendance at networking events and joining relevant professional bodies or groups.
  • To participate in corporate appraisal and quality management schemes.
  • To undertake any other duties as required to accommodate the changing needs of the service.
  • To comply with Tania Brown Ltd's equality policy and to ensure its operation within the context of the post, to include service delivery and employees issues.
  • To comply with the terms of Tania Brown Ltd's health and safety policy and procedures to ensure as an employee that the terms and conditions are met.
  • To be responsible for the implementation of Tania Brown Ltd's human resource policies and procedures including employee relations and training within supervisory responsibilities of the post.
  • To be responsible for all matters relating to the Data Protection Act within service delivery.

What you'll have:

The Ideal Candidate:

  • We require all applicants to be members of either NMC, HCPC, SWE, SWS or SWW.
  • All Case Managers at Tania Brown ltd must have a minimum of 5 years post qualifying experience.
  • Experience in working with clients suffering from spinal cord injuries and / or traumatic and acquired brain injuries and / or other complex conditions.
  • Applicants should have experience of working with clients in community settings.
  • It would be desirable for the candidate to have both adult and child experience, but this is not essential.
