Summary of Job
Provide social work services to OEHC patients and their families including counseling regarding the financial, social, and psychological aspects of occupational disease. Assess patient's needs, develop action plans directed towards resolving issues including crisis intervention, education about
illness and injury and common responses, financial issues, and referrals to community agencies and other services, as appropriate. Coordinate and facilitate non-medical or non-clinical care between patient, providers, and community resources.
Responsibilities- Assess EmblemHealth Health@Work patients' needs.
- Develop, facilitate and communicate a plan of care for patients with multiple medical, social and or mental health needs in partnership with the patient, his/her significant other, case manager, and providers to meet patient's needs.
- Continuously evaluate plan of care and revise as needed.
- Provide direct counseling and educational services to patients.
- Refer patients to community and other social service resources as appropriate.
- Collaborate with the SUNY Albany School of Social Services to develop and support a social work intern program within the Occupational Health clinic to support workers' holistic health.
- Participate with team members in grant writing and developing and implementing program policies and procedures for crisis prevention.
- Establish referral network/linkages with outside agencies.
- Bachelor's degree (Required)
- Master's degree in social work (LMSW) (Required)
- Certification in clinical social work (LCSW) (Required)
- Active, unrestricted New York State License to practice Social Work (Required)
- 4 - 6+ years' relevant, professional work experience (preferably in occupational health or related field) (Required)
- Case management and/or managed care experience (Required)
- Health plan experience (Preferred)
- Knowledge of Federal and State regulations for Medicare and Medicaid and other national and state funded programs (Required)
- Knowledge of NYS Workers' Compensation System regulations (Required)
- Knowledge of community resources access and ability to make appropriate referrals to internal/external programs that meet the patient's needs (Required)
- Ability to supervise social work interns (Required)
- Strong organizing and prioritizing skills; ability to successfully manage multiple tasks/projects simultaneously (Required)
- Must be detail oriented; ability to identify, analyze, and solve problems (Required)
- Excellent communication skills (verbal, written, presentation, interpersonal) with all types/levels of audiences (Required)
- Proficient with MS Office (Word, Excel, PowerPoint, Teams, Outlook, etc.) (Required)
- Ability to screen and stratify patients who are appropriate for services; manage a caseload of patients who need case management; create and execute case management care plans and document per EH policies and procedures (Required)
- Ability to engage and diffuse crisis situations; and to work effectively with patients, their families, professionals, and other
personnel from a wide range of cultural, social and economic backgrounds (Required)
