Job Summary:
We are currently seeking a Rehabilitation Support Worker to join our dedicated team working with a 24-year-old gentleman who has an acquired brain injury and polytrauma. The role involves providing support with therapy sessions, daily living activities and community engagement. Successful candidates will work under the guidance of the case manager, assistant case manager, and the multi-disciplinary therapy team.
The client is a very friendly gentleman with a great sense of humour. In addition to his rehabilitation, he enjoys fishing and would like his Support Worker to support him with this. He also enjoys listening to music, DJing and is interested in learning music production.
The ideal candidate will possess experience in a care or support environment, demonstrating assertiveness, a genuine passion for providing support, and the ability to engage in friendly banter with clients. Excellent communication skills are paramount, and a full clean driving license is a prerequisite for all applicants. This role requires individuals with a high degree of confidence, initiative, and capabilities in lone working. Candidates should embody trustworthiness and a naturally caring disposition.
Key Responsibilities:
· Implement the rehabilitation plan, therapy exercises, and guidelines.
· Support the client in accessing social and recreational activities.
· Drive the client to various venues and appointments.
· Perform light cleaning duties in the client’s home.
· Work under the instruction and supervision of the case manager and multi-disciplinary therapy team.
· Liaise with the case management team and report any concerns promptly.
· Complete detailed daily shift notes.
· Ensure timely completion of allocated training.
· Advocate for the client when necessary.
· Ensure the client's safety, meet his needs, and communicate any concerns to the management team.
· Attend team meetings, team training sessions, and monthly supervision meetings.
Qualifications and Requirements:
Essential
· Full clean driving license
· Access to own vehicle
· Clear DBS check
· Minimum of 1 year of experience in a care or support setting
· Excellent written and verbal communication skills
· Ability to problem-solve and work well under pressure
· 2 professional references
Desirable
· Experience working with, and/or knowledge of acquired brain injury and its effects on individuals
· Experience working with, and/or knowledge of polytrauma and its effects on individuals
