Job description

The primary purpose of this role is to deliver on-going education, advice and support for patients with Diabetes to enable and empower their own self-management. The post holder will be able to offer appropriate advice and support to a patient on a range of core diabetes related clinical management issues. To teach and support patients in the correct use of diabetes related technology and skills such as carbohydrate counting and insulin dose adjustment. Additional training in dietetic / diabetes nursing skills will be provided as needed as it is not envisaged applicants will have both skill sets. The post holder will consult with Nursing or Dietetic colleagues in relevant Diabetes MDT when specialist advice is needed. To work to meet the needs of patients and service users who struggle to attend hospital-based clinics such as by independently delivery out-reach clinics held in GP surgeries, schools, colleges, and community settings. To take a patient centred approach, adapting education and advice to meet the need of the service users. To work in a collaborative and in patient-centred manner with members of the Diabetes MDT to support Patients with all types of Diabetes. To encourage and empower patients to make full use of the available technology to assist them in their own self-management by facilitating home and out of hospital-downloading of glucose related diabetes devices. Will be required to undertake leadership roles within the MDT. To provide clinical advice and support to non-clinical members of the MDT (i.e. Youth Workers) Support the Trust Governance and Patient Safety agenda. Influence change and development of practice, utilising the most recent evidence base. Abide by Trust policies when dealing with complaints, accidents and incidents. Maintain discipline within the clinical area, abiding by Trust Disciplinary Procedure Policies. Participate and chair interviewing panels in the appointment of staff and be aware of legislation and local personnel policies affecting the employment of staff. Implementation of evaluation audits to measure the impact of educational programmes and disseminate the results. To deliver a competency-based induction, where appropriate, for new staff and students to the directorate. Identify areas of good practice and provide opportunities for staff to share this across the organisation. Please note it is a requirement of The Newcastle upon Tyne Hospitals NHS Foundation Trust that all successful candidates who require a DBS for the post they have been offered pay for their DBS certificate.The method of payment is a salary deduction from your first months pay.
