Manage a caseload of patients in agreement with the Community Services Manager with input from the consultants in palliative medicine and monitor own caseload and the level of intervention to ensure appropriate use of resources. Demonstrate leadership through clinical expertise, delivering high standards of holistic specialist level palliative care by acting as a team resource, identifying needs through a holistic needs assessment, and co-creating an Advance /Anticipatory care plan with the patient. Support carers and provide signposting information as appropriate to enable informed choices to be made concerning treatments and future care. Contribute to multi-professional discussions e.g. Gold Standards Framework meetings, MDT meetings, discharge planning, and case conferences. Facilitate meetings with the district nursing team. Provide bereavement support within the boundaries of own competency and role referring to other agencies as appropriate. When qualified as a NMP, prescribe a range of medicines in line with own prescribing formulary ensuring that this is cost effective and in line with St Catherines NMP policy. Complete relevant orders and procedures as applicable e.g. Verification of Death procedure, Unified Do Not Actively Resuscitate orders. Contribute to the provision of the 24-hour hospice designated advice line, which includes the opportunity to partake in an overnight on-call system for additional remuneration. Completion of accurate and contemporaneous documentation including records of care, referrals, and clinical notes using written and electronic formats. Contribute to the investigation of incident / accident reports as required. Contribute to the Hospice audit and education programme, including acting as a mentor to colleagues as appropriate. Lead on continuous improvement project work team projects as agreed. Provide those in development posts with mentorship, training, guidance and support with achieving competencies. Comply with legislation, Hospice policies, procedures and protocols. Participate in Hospice agreed research activity as required. Carry out other duties within your ability and within reason as required by the Chief Executive, Director of Nursing, Governance and AHPS, Community Services Manager.
