Clinical Nurse Specialist - Nutrition Nursing Team

To provide effective care coordination to an identified caseload of service users, ensuring the delivery of evidence based interventions to encourage safe discharge and overall successful outcomes. To provide appropriate individualised intervention to patients in relation to need, liaising with other members of INCS as required. To work in partnership with other professionals/agencies across the primary and secondary interface to ensure that patients needs are met. Identify and evaluate areas for potential service improvement, across the healthcare boundaries for nutrition. Supports the Head of Service to actively promote innovative clinical practice, which is effective in terms of clinical outcomes and evidence based. To coordinate your own workload , assessing and identifying need, delivering evidence based practice, and ensuring a continuous cycle of audit and review in order to manage risk, maintain quality standards, and ensure unmet needs are catered for. Takes personal responsibility for ensuring clinical skills to be able to manage PEG tube placement, placement of nasal bridles, enteral feeding tubes, and TPN / central line management is up to date, and has a high degree of clinical competence. Submits to annual peer review of these skills. To work with the Head of Service in the coordination of service delivery. To support the Head of Service through carrying out agreed delegated tasks and deputising for them as requested. To actively participate in service audits and review processes, taking appropriate action under the direction of the Head of Service
