Job description

A Vacancy at Lancashire Teaching Hospitals NHS Foundation Trust.
A very rare opportunity to work in the award winning Nutrition Nursing Team at Lancashire Teaching Hospitals!
You will ensure high quality nursing care is delivered to all patients who require both enteral and parenteral nutrition over a seven day service. This will include the assessment of patients suitability for artificial feeding, placement of a range of feeding devices and the management of TPN / HPN.
In addition, you will be independently running the nurse led clinic for community patients who require assessment for enteral feeding devices, replacement of enteral feeding devices and trouble shooting to avoid unnecessary hospital admissions.
This role also encompasses being the nursing link in a wider multi-disciplinary team in order to care for hospitalised patients who require enteral and parenteral feeding. You will work with a group of expert nurses, dietitians, speech and language therapists, surgeons and gastroenterologists to influence nutritional care across Lancashire Teaching Hospitals and the community.

With the support of the Head of Service for Nutrition, you will be responsible for ensuring the delivery of specialist nutritional practice across the Trust in a patient centred, clinically effective way in line with Trust policies, procedures and national guidelines.
You will ensure that national advice, standards, and guidance is translated locally in order to ensure that standards for patients who require nutrition support are met.
You will also be responsible for nursing practice in this specialist area, providing leadership, support, advice, guidance and education for patients, significant others and colleagues across the healthcare community.

• To provide effective care coordination to an identified caseload of service users, ensuring the delivery of evidence based interventions to encourage safe discharge and overall successful outcomes.
• To provide appropriate individualised intervention to patients in relation to need, liaising with other members of INCS as required.
• To work in partnership with other professionals/agencies across the primary and secondary interface to ensure that patients needs are met.
• Identify and evaluate areas for potential service improvement, across the healthcare boundaries for nutrition.
• Supports the Head of Service to actively promote innovative clinical practice, which is effective in terms of clinical outcomes and evidence based.
• To coordinate your own workload , assessing and identifying need, delivering evidence based practice, and ensuring a continuous cycle of audit and review in order to manage risk, maintain quality standards, and ensure unmet needs are catered for.
• Takes personal responsibility for ensuring clinical skills to be able to manage PEG tube placement, placement of nasal bridles, enteral feeding tubes, and TPN / central line management is up to date, and has a high degree of clinical competence. Submits to annual peer review of these skills.
• To work with the Head of Service in the coordination of service delivery.
• To support the Head of Service through carrying out agreed delegated tasks and deputising for them as requested.
• To actively participate in service audits and review processes, taking appropriate action under the direction of the Head of Service
