Summary: Role: Clinical Pharmacist (Primary Care) Setting: General Practice Location: Remote Salary: £45,000 - £60,000 FTE depending on experience Hours: 20 to 40 hours per week Job summary Join our remote clinical team as to help deliver best-in-class care to our patients. You would be joining a rapidly growing group of national GP practices providing primary care pharmacy support to deliver the highest standard of medication management, medicines optimisation and proactive care. You will play a large role in working with the wider team to help implement new initiatives and roll out innovation across our practices. Main duties of the job Provide best in class patient care to all our registered patients to improve health outcomes Support your colleagues and patients in rolling out and implementing new ways of working Help foster a collaborative, ambitious and innovative culture so we can build the best primary care service in the UK; a service which exceeds the expectations for patients and clinicians We would expect you to work within our pharmacy team to provide best-in-class care to our patients. Tasks would include: Prescription and medication queries Repeat prescription monitoring Medicines reconciliation Chronic disease management Structured medication reviews Quality improvement projects Other depending on your personal preferences It is essential that you are able to build working partnerships with the wider MDT, helping to motivate staff and maintain effective working relationships Excellent communication skills with all service users in a professional, efficient and polite manner is essential There is always the support of a senior clinicians and GPs to help you practise safely and further develop your clinical skills, competencies and scope of practice depending on the needs of our group and your own interests.
