The duties of the role may vary from time to time depending on current and evolving workloads and staffing levels:
- Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
- Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care
- Develop relationships and work closely with other pharmacy professionals across the wider health and social care system
- Provide structured medication reviews.
- Manage repeat prescription authorisations and reauthorisation and acute prescription request.
- Processing hospital discharge/clinic letters
Key requirements
Clincial Pharmacist requirements and qualifications:
- Able to work on own initiative and as an effective and responsible member of the team
- MSC qualification or equivalent
- At least six months’ primary care experience in GP setting over the last few years
- Professional pin registered
- Outstanding organisational and time management skills
- Ability to make decisions in high-pressure situation.
- Ability to work unsupervised on a day to day basis and to manage/ prioritise a delegated caseload independently
