Responsibilities :
- Collaborate with healthcare professionals to develop and implement medication therapy plans for patients
- Review patient medical histories and assess medication regimens for safety and effectiveness
- Provide drug information and education to patients, caregivers, and healthcare providers
- Monitor patient responses to drug therapy and make recommendations for adjustments as needed
- Participate in multidisciplinary rounds and provide clinical pharmacy services in various patient care areas
- Conduct medication reconciliation and ensure accurate medication orders
- Assist in the development of pharmacy policies and procedures

Essentials :
- Minimum of 1 year of UK experience as a Clinical Pharmacist in a hospital or clinical setting
- Strong knowledge of anatomy, physiology, and pharmacology
- Proficiency in patient care and medication administration
- Current DBS on Update Service/or able to Pass an application
- Excellent communication and interpersonal skills. - Registered with the GPhC -Fluent written and spoken English including good numeracy
