Job Description:As a Clinical Pharmacist at P2P Recruitment, you will play a crucial role in providing comprehensive pharmaceutical care to patients within healthcare facilities. You will work closely with healthcare teams to optimise medication therapy, ensure safe and effective medication use, and promote positive patient outcomes.

Responsibilities:

  1. Medication Management: Review medication orders and patient profiles to ensure appropriate therapy, dosage, and administration. Collaborate with healthcare teams to adjust medication regimens based on patient needs and clinical guidelines.
  2. Clinical Consultation: Provide drug information and clinical consultation to healthcare professionals, patients, and caregivers. Offer recommendations on drug therapy, adverse reactions, and medication interactions.
  3. Patient Education: Educate patients and caregivers on medication use, including proper administration, potential side effects, and adherence strategies. Empower patients to actively participate in their treatment plans and manage their health effectively.
  4. Quality Assurance: Participate in medication safety initiatives and quality improvement projects. Conduct medication reviews, monitor therapy outcomes, and identify opportunities for process enhancement to ensure the highest standards of pharmaceutical care.
  5. Interdisciplinary Collaboration: Collaborate with physicians, nurses, and other healthcare providers to optimise patient care and outcomes. Communicate effectively with interdisciplinary teams to coordinate medication management and resolve medication-related issues.
  6. Regulatory Compliance: Adhere to state and federal regulations governing pharmacy practice, including dispensing requirements, controlled substance management, and documentation standards. Maintain accurate records and uphold ethical standards in all aspects of pharmaceutical care.
  7. Continuing Education: Stay informed about new medications, treatment guidelines, and emerging trends in pharmacy practice. Engage in professional development activities, such as attending conferences, participating in continuing education programmes, and pursuing certifications to enhance clinical knowledge and skills.

Qualifications:

  • Bachelor of Pharmacy (B.Pharm) degree from an accredited pharmacy programme
  • Active pharmacist registration with the General Pharmaceutical Council (GPhC)
  • Clinical pharmacy residency or equivalent experience preferred
  • Strong clinical knowledge and understanding of pharmacotherapy principles
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively in a fast-paced healthcare environment
