An exciting opportunity has arisen to recruit a highly motivated, enthusiastic Advanced Practitioner who have attained or is working towards the Full MSC in Advanced Clinical Practice including the Non Medical Prescribing module (this post is at Band 7).
There will be a requirement to provide a high level of clinical expertise, knowledge and skills, taking a lead role with clinical assessment, diagnosis, treatment and management of patients who are experiencing an acute episode of their chronic condition, are an elderly medically frail deteriorating patient in the community and/ or a high intensity users of services. The focus will be to support patients to remain safely in the comfort of their own homes for longer, prevent hospital admissions, facilitate early safe discharge and reduce GP contacts.
The post holder will have or be working towards a full MSC qualification in Advanced Clinical Practice and be a highly skilled clinician with disease specific knowledge to provide clinical expertise and evidence based care in areas of clinical assessment, treatment, management of complex needs and evaluation of patients within the cluster population for example those who are experiencing an acute episode of their chronic condition/and or a high intensity user of services. The role will be based with the integrated Community Resource Team operating close working relationship with Primary and Secondary Care.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
