Community Support Worker

 

Hours: 37 hours a week

Salary:  £25,500

Location: Hybrid/Worthing

About The Role

Join the Prevention Assessment Team (PAT) as a vital member of our Multi-Disciplinary Team (MDT). As a Community Support Worker, you will play a key role in offering support and guidance to individuals who may not qualify for statutory services but still have needs. You'll engage in conversations about wellbeing, promote independence, and connect people with the right resources in a friendly and personalized manner. It's all about helping our community thrive.

This is a hybrid position with the option to work from home on a regular basis.

Main Duties:

  • Work closely with the PAT team to support individual health and wellbeing, offering early intervention and self-help advice through phone calls, letters, or face-to-face chats.
  • Follow the Gateway Model of PAT, ensuring cases are assigned to the right places and managing a caseload from the voluntary sector, including joint efforts and Community Led Support (CLS).
  • Assist individuals in accessing necessary health, social care, and wellbeing services, completing relevant paperwork as needed.
  • Listen to people's concerns, provide practical advice, and connect them with local services to improve their quality of life.
  • Build relationships with local providers and organizations to better support our community.
  • Act as a bridge between at-risk individuals and Health and Social Care professionals to prevent crises.

Qualifications and Skills:

  • Experience in Community Support roles.
  • Good communication skills.
  • Knowledge of local health and social care services.
  • Ability to work well with a team and other agencies.
  • Willingness to keep learning and grow in your role.
