The post holder will demonstrate the knowledge and skills of a competent practitioner and will: Be accountable for own practice in accordance with the NMC Code of Conduct and trust policies, recognising own limitations within professional boundaries. Be a part of a small team of 6-8 midwives exploring innovative ways of working Be responsible for the identification and assessment of women suitable for a Continuity of Carer pathways and implementation and evaluation of programmes of care in accordance with Better Births 2016 Work as a team member, both within the Hospital, Community and Birth environments and, where appropriate, as a member of the primary health care team Be responsible for the management of individual case load of women, providing midwifery care to an identified mixed risk, geographical based caseload of women, with co-ordination or referral of high risk women to consultant care, facilitating transfer between the two systems. Carry out appropriate screening tests on women and babies, following appropriate discussion with the woman and informed consent. Be responsible for managing own caseload flexibly to offer Intrapartum care to an identified caseload of women, managing on call commitments or birth availability depending on model utilised
