When taking charge of the ward/department or in the absence of the ward/department manager, is directly responsible for ensuring that clinical, professional and cleanliness standards are maintained in the entire ward/department. Manage the team to ensure timely discharge processes are followed and national guidance re discharge are met and delivered This job role includes daily liaison with clinical site managers, discharge nurses and other members of the Multi -Disciplinary Team to support the timely identification of patients for planned discharge on the day that they become community ready. This includes the Emergency Department Will work with both internal and external partners Manage the systems in place to assist the ward staff to prepare the patient for transfer into the discharge lounge this will include, washing, dressing, assisting with breakfast, collecting and bagging personal property, collecting medication from pharmacy and transferring the patient to the Discharge Lounge To comply with the trusts vision, values and behavioural compact.
