About the Reablement and Hospital Discharge Team:

Our team comprises of dedicated professionals committed to ensuring residents have a smooth and safe discharge from both hospital and community discharge services. We are committed to supporting individuals in their journey to recover and maintain independence following illness or injury. With our base in the community, our team operates in a fast-paced environment where each day brings new opportunities to make a difference. Your Professional Development:

We recently developed and implemented our new career progression framework to create long term stability in the social worker workforce by creating opportunities for progression and supporting professional development. We also provide formal development for experienced social workers by supporting them to undertake practice educator and AMHP training. We provide management courses to help you evolve as a manager.

Main Responsibility

  • As a Deputy Manager, you will play a crucial role in developing the team to meet the evolving needs of our service.
  • You will be instrumental in implementing changes to address the increasing demands of our residents.
  • This role involves managing social workers and social work assistants, fostering a collaborative and supportive environment.
  • You will work closely with multidisciplinary teams, contributing your expertise in Hospital Discharge, continuing health care and community support.​

The Ideal Candidate

  • Ideally you will have experience in Hospital Discharge and multidisciplinary teamwork.
  • You will care about making a positive difference, supporting team development all whilst delivering exceptional care to our residents.
  • Collaboration is key, and a demonstrable experience in building strong relationships is essential.
  • Due to the requirement to work in different locations in Milton Keynes, access to own transport is essential.
  • Applications will only be considered from those who are qualified Social Workers as registration with Social Work England is mandatory.
  • Driving licence - for this role you are required to have a full UK driving licence. If you are successful, you will be required to supply evidence of your licence prior to starting the role. You will also be required to provide evidence of your vehicle insurance with business use. This is required for any roles that requires the employee to travel to more than one place of work.

*Golden Hello - A one-off payment will be made to new recruits who qualify subject to the following terms being accepted. This payment will be pro rata for part time roles.

Our roles are either designated ‘site based’ or ‘home based’. ‘Site based’ roles can include up to two days a week working from home whilst ‘home based’ roles are based at home four days a week.  Of course, managers designate which category roles are in based on the needs of the service and the circumstances.
