Job description
The role of the Fundamentals of Care Assistant is to support registered professionals within the team to optimise outcomes relating to the Fundamentals of Care such as falls prevention, continence assessment and pressure ulcer prevention.
The post holder will demonstrate compassionate care to those they support and be able to use effective communication to uphold Trust values whilst promoting the Fundamentals of Care Team vision which has holistic care at its core.
To support registered members of the Fundamentals of Care Team in the delivery of a 6 day service which runs between 7:30 – 6pm inclusive of multi-site working.
Provide effective evidence-based care, which remains within their scope of practice and overseen by registered members of the team.
Play a key role in optimising and delivering holistic patient centred care which promotes the safe delivery of key aspects of the Fundamentals of Care such as falls prevention, pressure ulcer prevention and continence management.
Demonstrate all Trust values optimising the patient experience and Multi-Disciplinary Team working.
Will hold or will be willing to complete the Care Certificate, practicing within their scope of practice and promoting all standards within this.
Prioritise own workload demonstrating the ability to manage this independently, seeking support as required and reporting concerns to their line manager or nominated Nurse in Charge.

CUSTOMER CARE FOR PATIENTS AND/OR SERVICE USERS
To work under the guidance or supervision of the registered staff member.
Ensure all delegated tasks are implemented, ensuring progress or concerns are reported back to the registered staff member before moving on to next steps.
Assist the registered staff member with providing prescribed intervention / care task, including follow-up visits to patients and making requests to clinical areas.
Prioritise and promote patient independence and recovery through holistic care delivery.
Act as a support to patients, providing education to them within their scope of practice.
Respect privacy, dignity, autonomy, and the less restrictive approach to care delivery for all.
Hold a clear understanding of referral processes to the team and pathways where clinical areas may be sign-posted.
Demonstrate understanding of Trust values promoting organisational and local visions.
Contribute to a clinical and team environment which is positive and inclusive of all.
Contribute and support registered staff in audit completion, data caption or other tasks which improve service delivery or contribute to effective care delivery.
Work within scope of practice at all times, which may include basic dressing changes, basic nail care delivery or supporting the registered staff member with any required interventions.
Remain in date with all mandatory training embedding this into practice.
Contribute to service development and progression.
Demonstrate awareness of local and organisational policy.
Work within the team and wider Multi -Disciplinary Team demonstrating effective communication at all times.

Supports the registered staff member with the delivery of education across the trust i.e. trolley dashes
Supports registered staff members with daily admin tasks such as data entry
Supports team members with any research and or trials that they are participating in
