We are looking for a Health & Wellbeing Coach with a background in exercise/ physical activity to work at Newport Health Centre supporting patients with chronic pain.
This role will be based at Newport Health Centre; We have 2 practices in Newport (The Dower House Surgery & Carisbrooke Surgery). We have approximately 24,500 patients registered at our practice and pride ourselves on providing exceptional care to our community.
We have an extensive multi disciplinary team that will provide peer support for the delivery of this role; You’ll work closely with GP’s, Clinician’s and additional roles which include our Social Prescriber, Care co-ordinators, Dietician team and our Mental Health practitioners.
This role would be suitable for an individual that meets the below criteria:
- Are you an exercise professional or do you have a clinical background in helping people with managing pain?
- Are you passionate about helping people overcome barriers to achieve their goals?
- Are you a qualified exercise professionals OR clinical/ support staff with a basic clinical knowledge of osteoarthritis and its management, ideally including familiarity with delivering education and exercise to patients

In order to attend this training and apply for the role: You will need to demonstrate you meet the pre-requisites: Facilitator training – ESCAPE-pain

Main duties of the job
- The role is varied, and will include supporting self-management education, peer support, case management and facilitating group consultations, as well as the delivery of Escape Pain programme and facilitation of other exercise approaches.
- You will support personal choice and positive risk taking, while ensuring that patients understand the accountability of their own actions and decisions.
- This exciting role is to support patients with chronic pain to develop evidence based strategies to live fulfilling and enriching lives. The evidence is overwhelming that the right type of exercise and lifestyle approach is key to improving patients situations.
