Job description

We are looking to recruit a motivated and driven Lead Advanced Clinical Practitioner. They will be a senior Advanced Clinical Practitioner who will play a lead role in the development, management and governance of ACP roles across Acute Medicine, Frailty and specialist medicine. The role will also provide professional leadership to the out of hours nurse practitioners and DVT specialist service.
The role has designated 40% direct clinical time working in Acute Medicine and 60% non-clinical time. The Acute Medical Unit is a busy department comprising of an SDEC, short stay unit (CDU) and medical admissions ward. The Acute Medical Team are a busy and dynamic speciality who excel in delivering high quality patient-centred care across the Acute Medical Unit. The unit is in the process of further development and there will be opportunities be involved in service development, audit and research.
The successful applicant will be a Registered Nurse with Valid NMC registration or Health Care Professional and registered with HCPC. They will already possess a Relevant Masters’ degree in Advanced Practice or have a recognised qualification via the supported portfolio route from HEE Centre for Advancing Practice. They will also have evidence of extensive professional/clinical knowledge of Acute Medicine supplemented by specialist clinical training and continuing professional development.
This is a non-medical prescribing, fulltime post (37.5hrs/week).
The ACP Lead is key member of the Acute Adult Management Team. They are a senior Advanced Clinical Practitioner who exercises expert levels of clinical judgement, decision-making and leadership, and plays a key role in the delivery of the Trusts strategic vision. As an expert in their clinical area, the post holder will practice at a highly specialised and skilled level and will influence improvements in care and patient outcomes. The ACP Lead will be an expert resource, playing a lead role in the development, management and governance of ACP roles across Acute Medicine, Frailty and specialist medicine.
The ACP Lead will be based within Acute Medicine on the acute medical assessment areas at the Royal Bolton Hospital and will also act as professional lead for the out of hours nurse practitioner service and the specialist DVT service. They will develop and establish credible working relationships at all levels of the organisation and work alongside the senior divisional leadership team to support in the transformation of the ACP workforce.

The Acute Adult Care Division is one of four Clinical Divisions in the Trust that includes Emergency Care, Complex Medicine and Specialist Medicine. The Division contains the busiest single site Emergency Department in Greater Manchester and has inpatient wards providing acute medicine, complex and specialist medicine services. It provides a range of specialist out-patient and community services and is committed to providing excellent clinical performance and patient care.
The post holder will:
• Have a relevant Masters’ degree in Advanced Practice or recognised qualification via the supported portfolio route from HEE Centre for Advancing Practice.
• Have a valid NMC registration or be a Health Care Professional and registered with HCPC with relevant clinical experience.
• Have a highly visible clinical profile in Acute Medicine, providing both advisory and direct clinical input into patient care.
• Manage a defined caseload of patients, including transfer and/or discharge from care with distant supervision from the Medical Consultant body.
• Be able to analyse, synthesise and action highly complex clinical information and situations to benefit the patients and families.
• Demonstrate clinical leadership, guidance and judgement in making challenging patient centred decisions, identifying clinical situations that need escalating to the Consultant team.
• Recognise, understand and accept the requirement to act as an advocate for patients, carers/relatives, the service and the organisation.
• Develop and maintain expertise in Advanced Clinical skills and competencies in line with Trust and NMC or HCPC regulatory frameworks.
• Demonstrates expert interpersonal skills in caring for patients and their families.
• Work collaboratively with multidisciplinary teams to optimise patient care.
• Lead on audit processes in Acute Medicine that will facilitate and improve patient flow including integrated care pathways and clinical guidelines.
• Identify and implement new developments, treatments and technologies into clinical practice Trust wide.
• Promote the translation of evidence based research findings and best practice guidance into clinical practice to improve the health of the local population.
• Maintains accurate and concise clinical records.
Leadership and Consultancy:
The post holder will be expected to:
• Act as a role model in delivery of patient care that reflects the Trusts strategic vision, values and behaviours.
• To professionally lead the ACP’s in Acute medicine, Frailty and Specialist Medicine, and the nurse practitioners in the out of hours service and DVT specialist service. This will include supporting their personal development as well as helping guide and develop their roles within their specialities.
• Be influential and innovative in shaping the development of the Acute Medicine and SDEC services, leading and engaging where appropriate with external agencies and commissioners.
• Be an active member of relevant Trust forums and committees to provide expert clinical advice and contribute to the development of multi-professional policy and strategy.
• Work with partner organisations, agencies, forums, voluntary sector and integrated health care to contribute to policy and strategy.
• Adhere to professional standards of care and promote a professional image of health care professionals that inspires confidence in service users, their carers’ and relatives.
• Participate in Trust Wide Divisional reviews, serious untoward incidents, Structured Judgement reviews and HR reviews; producing the written accounts/reports where required.
• Work collaboratively and strategically with other clinical areas in promoting patient satisfaction to provide a seamless pathway of care.
• Interpret, adapts and implements national policy and standards to enhance patient safety and good quality care.
• Lead in the development of documentation that is consistent with legislation, policies and procedures.
• Lead on change management and the impact of change.
• Maintains awareness of budgetary constraints within the department, ensuring appropriate use of resources in order to meet service/patient needs.
Learning, developing, improving across the system
The post holder will be expected to:
• Act as a coordinating/associate supervisor for trainee and qualified ACPs in Acute Medicine.
• Lead on the development of ACP roles within acute medicine, frailty and specialist medicine including support with centre endorsed competency frameworks.
• Lead on the development of specialist nursing roles in the out of hours service and DVT specialist service.
• Act as an expert source of clinical and theoretical knowledge for junior members of the acute medical team.
• Develop methods of peer support and clinical supervision in line with NHSE guidance.
• Works with the MDT to further develop clinical pathways and care approaches.
• Develop health promotion and education in collaboration with other health care professionals to ensure that care and resources reflect current and best practice.
• Ensure the clinical learning environment is conducive to supporting the education and learning of qualified staff and trainees.
• Work closely with colleagues in local higher education facilities in developing, planning and delivering educational programmes for staff to improve knowledge and skills in caring for patients.
• Take responsibility for their own learning and performance including participation in clinical supervision and annual performance review.
• Maintain professional registration and CPD requirements in line with professional body guidelines.
• Leads or participates in educational events at local, regional and national level to share best practice and collaborative working.
