Mental Health Support Coordinator 2x roles

We have an exciting opportunity for 2 Mental Health Support Coordinators to play a vital role in providing support for our Emotional Wellbeing Service.

Position: 2x Mental Health Support Coordinator

Location: Hybrid roles with a mix of homeworking and within GP surgeries across Haywards Heath or Burgess Hill

Salary: £25,948

Hours: 37 hours

Contract: Permanent

Closing Date: 24th March 2024

Interview Date: 2nd and 3rd April 2024

About the Emotional Wellbeing Service:

This is an amazing opportunity to join an incredibly supportive team of people who provide Mental Health Support to patients, directly within GP practices across Mid-Sussex and Crawley. The Emotional Wellbeing Service works with patients to create personalised support plans and deliver person-centred support using a range of grounding techniques, coping skills, CBT tools, goal setting and signposting. We also work in partnership with Sussex Partnership Foundation Trust and our clinical Mental Health Practitioners who work alongside the Mental Health Support Coordinators to provide clinical interventions for patients who may present with higher needs.

About the role:

We have two vacancies for this innovative role which will be based in GP surgeries in the Haywards Heath and Burgess Hill area. The exciting role of a Mental Health Support Coordinator is to effectively meet the needs of patients who are experiencing Mental Health challenges, through the provision of direct phone, video-call, and face to face support and by encouraging these patients to access and engage with community services. You will be joining a passionate team who work together to support individuals with Mental Health needs and empower each other to develop their own knowledge and skills. You will also have 1:1 support, group reflective practice and debriefs with a Mental Health Practitioner who provide clinical advice and guidance and support your own development of skills and knowledge of Mental Health issues and pathways.

You will be committed to delivering high quality preventative Mental Health interventions and will have experience of working with vulnerable people using person centred and empowering approaches. Excellent communication and organisational skills are essential as a Mental Health Support Coordinator along with energy and enthusiasm to support and empower patients to better manage their issues and create personalised support plans to achieve attainable goals.

You will be a part of the Adult Community Mental Health Transformations Teams initiative, "To increase understanding of Mental Health and wellbeing in our communities and make it easier and quicker for people to access the support they need, when they need it, close to home." as part of the NHS Long Term Plan.

Key Responsibilities:

  • Provide timely telephone, video call and face to face Mental Health Support to patients identified by the practices as requiring this, including assessing needs and risks using the Pathfinder framework, paperwork, and processes.
  • Support patients to identify their needs and goals and facilitating the development of personal support and recovery plans with patients.
  • To develop, establish and maintain a regular and agreed presence at several specified GP surgeries working in partnership with key practice staff to do this.
  • Liaising regularly with and referring any clinical needs or issues to the Emotional Wellbeing Service Clinical team particularly ensuring any presenting risks are fully discussed and addressed.
  • Accurate and up to date record-keeping of work using agreed systems within practices and BHT Sussex and to report on work activity as requested.

Person Specification:

To be successful in the role of Mental Health Support Coordinator you will need to have great communication skills and be committed to the values of the organisation. Some of the key skills and experience you will need to bring with you include:

  • Broad knowledge, proven skills and/or training which relates to supporting adults who have Mental Health needs, using appropriate theories, methods, and skills to promote service users’ ability to better manage their challenges and create personalised support plans to achieve attainable goals.
  • Thorough understanding and ability to execute professional boundaries and the importance of this when supporting vulnerable adults and working in a hybrid working environment to ensure a harmonious work/life balance.
  • Excellent computer and organisational skills and ability to manage own diary alongside a steady caseload of patients, whilst working in a busy GP surgery environment is essential. Ability to demonstrate time management skills and independently organise and prioritise own workload, ensuring patient information is recorded on referral systems in a timely and efficient manner.
  • Knowledge and experience of working within a team environment, supporting colleagues, and developing and maintaining positive key relationships with external and internal professionals
  • Willingness and commitment to travel across different sites for meetings and appointments and be flexible with meeting patients via telephone, video call or face to face at GP practice’s and within a community setting.
