Grove Medical Centre in Felixstowe has a fantastic opportunity for Nurse Practitioner to join our team on a full or part-time basis, depending on the successful applicants requirements, to provide a service to over 16,500 members of the surrounding community.
You will become a part of a forward-thinking practice which has significantly evolved during these times of change, to enable us to provide excellent patient care, whilst protecting & supporting our valued team.
We are looking to further expand our existing nursing team and you will provide clinical leadership to our nursing and HCA staff, and we are therefore looking for someone with previous experience in a similar role, or someone with the willingness and ability to develop into a leadership role and who is able to demonstrate the required skills and qualities for this post. The clinical team is very well supported by an experienced and welcoming management and administrative team.
The main responsibilities of the role include:
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
- Meeting the needs of patients presenting for opportunistic wound care.
- Hypertension review and management.
- Running asthma, COPD and diabetes clinics and reviews.
- New patient and health checks.
