Alexandra and Crestview Surgeries have a fantastic opportunity for an experienced nurse practitioner or Advanced Nurse practitioner to join our busy but friendly team on a full-time basis. As a nurse practitioner, you will take on a dedicated and important role. You will run telephone triage clinics in the surgery, as well as have a patient-facing role, you may also undertake home visits.
The main responsibilities of the role include:
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Meeting the needs of patients presenting for opportunistic wound care.
- Hypertension review and management.
- Running asthma, COPD and diabetes clinics and reviews.
