About the Leeds Outreach team
We oversee semi-independent accommodation and outreach services covering Yorkshire County, managing 30+ properties in cities like Leeds, Bradford, Sheffield, Rotherham, and Hull, with ongoing expansion. We support 40+ vulnerable young individuals at home, imparting skills for independent living. Our focus includes delivering ASDAN, addressing essential independence skills. We operate within a disciplined team, ensuring optimal outcomes through regular updates and active participation in statutory meetings. Our referrals come from trusted sources such as Local Authority placements teams, including Care Leavers Service, UASC, and Adult Services.
Due to the areas this role covers a car driver with a vehicle is essential.
About you
The ideal candidate will be passionate about this sector, looking to make a difference with our service users and will be looking for a challenging but rewarding role.
Essential
- Previous experience supporting (not caring) vulnerable people, families or young people to gain independence.
- Travel is essential so must hold a full driving licence and have access to a vehicle insured for business use.
- Must be prepared to travel throughout Yorkshire including Hull
- Thorough knowledge of safeguarding vulnerable adults and children
- Have or be willing to work towards an NVQ Level 3 Health and Social Care qualification.
- Computer literate
- Good interpersonal skills
Key Duties:
· Managing a caseload of vulnerable Service Users, helping to build independent living skills
· Liaising with relevant professionals such as Social Workers, Healthcare practitioners and the Police
· Writing detailed reports which assess a service user's independent living skills
· Completely weekly inspections of Service User's properties
· Support with accessing and maintaining education/employment
· Promote physical, mental and emotional well-being, de-escalating challenging situations when they arise
· Signposting to appropriate services as required, including mental health, drug rehabilitation services, counselling and education
