Job description
To Join the expanding physiotherapy interventions team within the community setting in Fair Havens Hub, IPU and patients' own homes in the community. Utilising and promoting a rehabilitative palliative care approach.
To support patients and those important to them with a palliative life-limiting condition.
Managing a caseload, using evidence-based/patient-centred principles within physical and psychological medicine to maintain independence and promote wellbeing.
Communicate highly sensitive information to a wide range of professionals as part of the multidisciplinary team around the family. Contributing to care plans and providing clinical observations to key professionals to safeguard and ensure that patients' holistic needs are met
To deliver an individual Physiotherapy treatment programme based on a sound knowledge of evidence-based practice and treatment options, using clinical assessment, reasoning skills and knowledge of treatment skills
Determine an individualised support plan incorporating appropriate members of the MDT and considering the patient's hopes, wishes and need
To advise and educate patients and their families/carers on the management of individual problems incorporating individual patient focused goals and outcomes on independence and quality of life
To work within the National Guidelines of NICE and relevant national government policies

This can be in the comfort of their own home, in the hospices – Fair Havens for adults or Little Havens for children - or a combination.
