Hoursperweek - 37 hours per week
Salary - £15.92ltd per hour
Southampton City Council are currently looking for a placement coordinator to provide support to the Adult Social Care Teams and Clinical Commissioning Group with tasks relating to sourcing support packages and placements and ensuring that individual contracts are in place.
Description:
- As a placement coordinator you will receive referrals from practitioners and secure appropriate support packages/placements.
- Placement officers have access to the social care records of vulnerable adults. To also have direct contact where appropriate with vulnerable adults and/or their representatives to advise on details of support secured for that individual and or/provide guidance on the actions the Placement Service will take to secure that support.
- Placement coordinators accurately create individual contracts, to enable payments to be made to providers within agreed timescales. These may include one-off arrangements, or amendments to existing arrangements.
- As a placement coordinator you must be able to organise and prioritise own workload to ensure that all contracts or arrangements are created promptly to enable payments to be made to providers within contract terms.
What is required?
- Educated to GCSE grade A-C in both Mathematics and English
- Previous adult social care experience
