Essential:
- Experience in working with people
- Excellent listening and communication skills
- IT skills in Word, Outlook, Spreadsheets,
- Zoom
- Good level of education
Desirable:
- A good knowledge of Self-directed Support (SDS) and current developments
around SDS
- Experience of working in partnership with Local Authority social work teams, care
providers and other third sector organisations
- An understanding of person-centred planning and outcomes
- Have an awareness of transitions issues, particularly from Children to Adult services
Job Description
- Act on referrals for pre-assessment from social work teams giving SDS information to people
who are waiting for a social work assessment or clients who are awaiting a review of their care
- Carry out one to one pre-assessment home visits/telephone/zoom calls
- Record referral information and feedback to referral source where appropriate
- Keep up to date records on number of referrals and source of referral etc
- Respond to initial (or one-off) queries about Self-directed Support
- Gain knowledge of Self-directed Support options available locally
- Follow-up on correspondence
- Identify and contact support groups/organisations and agencies to expand referral sources
and explain the 4 SDS options
- Offer support to children moving into Adult Services (Transitions)
- Distribute publicity where required
- Attend SDS Forum and provide updates on the pre-assessment project
- Support expansion of the SDS Forum by promoting membership
- Attend SIRD team meetings
- Collate referral information and contribute to SIRD pre-assessment project 6 monthly reports
to Inspiring Scotland
- Attend SDS Improvement Group meetings and other relevant meetings/forums
This post is for 30 hours per week, flexibility would be good in as much as some weeks will
carry a larger workload than others. Training will be provided as required. Salary is £25740
p.a. mileage will be re-imbursed at 45p per mile. Annual leave is 5.6 weeks pro-rata.
