Job Title: Social Worker - Older People Mental Health TeamLocation: SloughSalary: Up to 44,470 per annum

Responsibilities:

• Conduct comprehensive assessments of the Mental Health and social care needs of older adults within the community.
• Develop and implement personalized care plans in collaboration with service users, their families, and other professionals, ensuring a holistic approach to care.
• Provide therapeutic interventions and support to promote the Mental Health and wellbeing of older adults, enabling them to maintain independence and quality of life.
• Work closely with partner agencies, including health services and voluntary organizations, to coordinate support and services for older adults with complex needs.
• Advocate for the rights and interests of older adults, ensuring their voices are heard and respected in decision-making processes.
• Maintain accurate and up-to-date records in accordance with statutory requirements and council policies.
• Participate in team meetings, case conferences, and training sessions to enhance professional development and service delivery.

Requirements:

• Qualified Social Worker registered with Social Work England.
• Substantial experience working within Mental Health services, preferably with older adults.
• Sound understanding of the Mental Capacity Act, Care Act, and other relevant legislation and policies governing social work practice.
• Strong assessment and intervention skills, with the ability to formulate and implement effective care plans.
• Excellent communication and interpersonal skills, with the ability to engage effectively with service users, families, and multidisciplinary teams.
• Commitment to promoting equality, diversity, and inclusion in service delivery.
• Ability to work collaboratively within a multidisciplinary team and independently when required.
• Full UK driving license and access to a vehicle for travel within the community.
