Position Overview:
As a Hospital Discharge Social Worker, you will be a vital part of the health care team, working collaboratively to assess patient's needs, develop discharge plans, and facilitate a seamless transition from the hospital to their homes. Your role will involve close coordination with health care professionals, patients, and their families to address social, emotional, and practical aspects of post-hospital care.
Responsibilities:
- Conduct comprehensive assessments of patient's social and emotional needs prior to discharge.
- Collaborate with health care teams to develop individualised discharge plans.
- Provide counselling and support to patients and their families during the transition process.
- Coordinate with community resources, home care services, and other agencies to ensure a continuum of care.
- Advocate for patient's needs and rights in collaboration with health care providers.
- Educate patients and families about available resources and support networks.
- Maintain accurate and up-to-date documentation of interventions and plans.
Qualifications:
- Bachelor's or Master's degree in Social Work or a related field.
- Relevant experience in a health care setting, preferably with a focus on hospital discharge planning.
- Knowledge of community resources, post-acute care options, and social services.
- Strong communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary health care team.
- Compassion, empathy, and a commitment to patient care.
