Job description


The post holder will work within the discharge to assess (D2A) and home pathway teams. The post holder will be facilitating hospital discharges, liaising closely with hospital teams, assessing the needs of people when they are home; putting in a package of care, equipment and any therapy or nursing needs over a 6 week period. The post holder will work closely with stake holders to ensure that the person is supported safely at home and that there is a seamless transfer of care and support to other agencies when discharged from D2A and home pathway.
The post holder will collaborate with members of the multi-disciplinary, multi-agency team to ensure that patients individual needs, autonomy and independence are promoted.
The post holder will be responsible for implementing agreed nursing policies and clinical standards in accordance with agreed criteria and NMC Scope of Professional Practice, and monitoring the compliance of staff in accordance with agreed criteria.
The role of the post holder will be flexible the service is a Monday- Friday 8-8 service comprising of a few shift patterns and weekend working.
To work without direct supervision in the community. To triage referrals from acute hospitals/GP's and other services, manage and co –ordinate referrals and patient flow, liaise with hospital and social care to facilitate safe discharge home and ensure packages of care are in place when appropriate, order collect and install equipment for patient use in their own home, assess, plan implement and evaluate goal or treatment plans for patients when they are home, refer on to multi agencies and other stake holders.
To manage, support, supervise junior staff and students. To deputise for the team leads as required. To manage the caseload and service in the absence of the team lead.
To share and cascade any knowledge gained and ensure that all care and treatment are delivered using best practice-based research and to ensure that D2A/home pathway therapy colleagues are competent in any basic nursing care or assessments
