Hospital Senior Social Worker Grade 9

You will work as part of an integrated multi-disciplinary team in the Hospital team to provide high quality social work practice; ensuring individuals have a solution focussed plan towards achieving their goals and maintaining their independence. You will work creatively with individuals, local communities and our partners, empowering them to deliver the best possible outcomes for people and to create a positive Adult Social Care culture.
We are looking for individuals who can demonstrate:

- A capacity for honest reflection and openness to learning;
- Ability to establish professional, effective working relationships with a range of partners/colleagues, adults, families and their carers
- An individual who can support and help drive the new Discharge to Assess model once it is established.
You must be a qualified SOCIAL WORKER and be registered with Social Worker England. This post will provide and support expert and effective social work practice in complex situations, assessing and managing high levels of risk and providing expert opinion in relation to decisions surrounding complex cases. You will undertake a professional leadership role taking responsibility for the learning and development of others, through leadership and management, supervision, mentoring, assessing and teaching.
The Hospital team supports all individuals requiring longer term adult social care input on discharge from hospital; the SENIOR SOCIAL WORKER role is the main contact for multi-disciplinary working and needs to be in the office on a rota basis as part of an IDT supporting referrals and pathway decision making, whilst working with health professionals to assess their immediate needs, identifying the goals they would like to achieve, and the support required to meet these, working with Brokerage to identify suitable providers; ensuring a timely and coordinated discharge from hospital. Once the new discharge to assess model is in place it may mean a change in working practice, as well as location so the successful candidate must be flexible and have a good ability to adapt to change.
This post is a 7 day working post and may be based either in the hospital team or in the community assessing individuals post discharge and you may be required to work between the hours of 8am and 8pm.
