Job Title

Senior / Support Worker (Supported Living).

Overview

TPR Care Services supports young care leavers aged 16 years and over with their support needs and the transition to independent living.

This support can be short or long term.

Support Workers help Young People to manage their tenancies and live successful independent lives of their choosing in the community.

Location

Required to join our Young People and care leavers Supported Living accommodation in Droylsden and Oldham.

Please note this is support work. Not a personal care/carer role.

Essential Skills, Knowledge and Experience:

  • NVQ Level 3 (or higher) Working with children and Young People or equivalent. (Training can be given to the right candidate but all other essential skills must be met).
  • Relevant sector work experience.
  • Must have experience working with Children in a residential or Supported living setting.
  • Must have an enhanced DBS certificate and be willing to apply for a new one.
  • Must be willing to do further training to reflect the level of responsibility and the needs of the Young People.

Key Responsibilities:

  • To be familiar with and comply with TPR Care Services policies and procedures.
  • At all times maintain confidentiality about the post you are employed for, both during work time and out-of-work hours.
  • Maintain safeguarding standards at all times.
  • Maintain and provide up-to-date information using TPR recording systems in operation.
  • Maintain a smart dress code and always wear your TPR identity badge when at work.
  • Complete mandatory training and any other relevant courses for continued personal development.
  • Communicate effectively with colleagues and Young People.
  • Build supportive and trusting relationships.
  • Develop and maintain effective communication.
  • Hold regular team meetings.
  • Ensure relevant parties involved in the planning and review of support are regularly updated with changes and progress through regular planned reviews and reports.
  • Lead and support development within the team and provide coaching and mentoring to team members.
  • Work proactively with other members of the team to manage the service caseload and administrative responsibilities.
  • Assist in preparing, maintaining, and updating high-quality, individualised Support Plans.
  • Assist in setting personalised goals and achieving outcomes within the Support Plans.
  • Coordinate the provision of high-quality individualised support given by support staff.
  • Ensure good joint working practices are established and maintained.
  • Have an initiative-taking approach to achieving a positive, engaging, promotional and relationship-building role with those we care for, work with, and outside agencies.
  • Ensure rotas are in place to meet provision and effective support.
  • Support staff in the delivery of service and provide direct supervision.
  • Lead staff in the writing, implementing and reviewing support plans which reflect the needs, interests, and wishes of the individual.
  • Identify staff training needs and plans to meet these in conjunction with the Service Lead Manager.
  • To work on shifts rota to meet the needs of the service as agreed.
  • Promote and support inclusion.
  • Effectively identify and contribute to learning and development activities including meeting organisational targets.
  • Senior Support Workers will be required to participate in on-call requirements.
  • Ensure ongoing risk assessments are conducted and the risks are managed appropriately.
  • Provide support, including budgeting, job searches, education, benefits, daily living activities and emotional support. This may include practical assistance where skills are not yet developed.
  • Support individual educational/training needs, career goals and plans.
  • Reinforce links with local authority organisations.
  • Support individuals to ensure they receive all the assistance they are entitled.
  • Cleaning and basic household tasks.
  • Undertaking any other duties consistent with the grade and nature of the post as assigned by the Service Lead/ Manager.
