SalarySocial Care Coordinatorrequired to work with our client the Belfast Trust. This post will be based in the Shankill Wellbeing and Treatment Centre with hours of work being 9am – 5pm Monday to Friday. The rate of pay will be £12.86 per hour and this post will initially last for 3 months with the possibility of extension.
The main duties of this role will require you to work under the supervision of the social work team to implement care plans, maintain individual caseloads, provide advice and support to elderly patients and their families about available resources, and provide reports and information. You will also interact with other healthcare professionals regarding referrals and further treatment plans for the patients.
Essential Criteria:
- 5 GCSE’s grade C or above OR NVQ Level 3 in a care related subject
- 1 years’ experience working in Social Care OR Hospital setting
- To be NISCC registered (or willing to apply)
- Be willing to undergo an Access NI Check
- Willing to attend Mandatory Training – Free Of Charge
- Full clean driving licence
