We are seeking a dedicated and compassionate Social Worker to join our team. As a Social Worker, you will be responsible for providing support and assistance to individuals and families in need. Your role will involve assessing clients' needs, developing care plans, and connecting them with appropriate resources and services. Duties:
-face to face CHC MDT and D2A reviews. Initially in the Central area. This is a county wide role and there may be an expectation to travel to different parts of Lancashire
- Conduct assessments to determine clients' needs and develop individualized care plans
- Provide counseling and emotional support to clients
- Connect clients with community resources such as housing assistance, healthcare services, and financial aid programs
- Collaborate with other professionals, such as healthcare providers and educators, to ensure comprehensive care for clients
- Advocate for clients' rights and assist them in navigating complex systems
- Monitor clients' progress and make necessary adjustments to care plans as needed
- Maintain accurate and up-to-date documentation of client interactions and services provided
Qualifications:
- Bachelor's or Master's degree in Social Work or related field
- State licensure or certification as a Social Worker (if required)
- Experience working with children, families, or individuals in need
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Knowledge of community resources and social service programs
- Excellent problem-solving and decision-making abilities
Job Type: Temporary contract
Contract length: 6 months
Pay: £35.00-£42.20 per hour
Expected hours: 35 per week
