An exciting opportunity has arisen for a qualified Social Worker to join the Resilience Services.
We are looking for a caring, experienced or newly qualified Social Worker to work in our Home to Decide Team, dedicated to supporting our local residents following hospital discharge within South Gloucestershire.
You will help improve the quality of services provided to adults with care, working closely within a team of professionals to ensure that agreed performance and quality standards are achieved across the team and that working practices meet statutory and local requirements that optimise outcomes for service users.
Salaries are offered at points within these bands depending on experience:
- Social Worker: £33,945 - £36,648
- Advanced Social Worker: £38,223 – £41,418
To be considered for Advanced Social Worker evidence must be clearly detailed in your application.
- You will hold cases and be responsible for carrying out Care Act assessments with adults following Pathway 1 hospital discharges.
- You will work closely with the Home to Decide Care Responders to write reablement support plans to motivate service users to regain confidence, achieve goals and re gain independence as far as possible.
- You will review service plans and work alongside Responders, giving guidance, advice and feedback as appropriate.
- Using your knowledge and expertise you will work as part of the multi- professional team.
- You will ensure cases are managed within accordance with the Safeguarding procedures for vulnerable people.
- It will be your responsibility to ensure records are kept in accordance with professional standards.
- You will work within the Mental Capacity Act undertaking assessments of capacity and best interest decisions where required.
As an Advanced Social Worker you will be required to:
- support students on placement as an experience Practice Educator
- be fully up to date with the core training expectations as an Advanced Social Worker
- use examples of research and evidence-based resources to enhance practice and share learning with team (e.g. use of Research in Practice / other online resources).
What we need from you
- You will hold a relevant professional qualification e.g Social Work Diploma or equivalent and be registered with the Social Work England.
- You will have relevant post qualifying experience in a social care or health setting or possess transferable skills and knowledge.
- You will be able to provide evidence of on-going continuous professional development, knowledge of current legislation and practice relating to social care with the ability to be able to assess and manage risk.
- It is essential that you can prioritise your work, meet deadlines and work under pressure.
- We will require you to have a minimum of 2 years post qualification experience.
- You will have evidence of meeting the essential (and desirable where possible) criteria for the role of Advanced Social Worker or in South Gloucestershire council.
- A clear commitment to Continuous Professional Development (CPD)
Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post.
