We have an exciting opportunity available for a Level 2Social Worker on a fixed term contract for 6 months in the Older Adults and Dementia Team for the south of the county.
About the Team and the Role
This is a unique opportunity for a Social Worker to be part of an integrated multi-disciplinary specialist mental health team. The successful applicant will be employed by Warwickshire County Council but seconded to Coventry and Warwickshire Partnership Trust (CWPT) Mental Health Services under a successful section 75 agreement. This allows for the meaningful integration of the social care agenda within the local Mental Health Trust and prioritises a holistic person-centred approach to meeting the needs and outcomes of the local population.
You will play a crucial and valued role in both the Dementia and Memory Assessment Service, and the newly formed Older Adult service. The Dementia team consists of clinicians from different professional backgrounds skilled in assessing people with dementia, although you will be line-managed by one of our social care Team Leaders. The team also provides post diagnostic information and support for people who have just been given a diagnosis of dementia and are skilled at managing often complex and challenging cases which require specialist input.
As part of the Older Adult service, you will take a strengths-based approach to supporting people with a range of mental disorders including schizophrenia, depression, anxiety, OCD and personality disorder; maximising their potential for independent living and promoting their full inclusion in the community.
