Working alongside South Central Ambulance Service (SCAS) in their call centre Monday to Friday, you’ll work closely with ambulance crews who attend to older people in crises in their own homes, with the aim to reduce admission into Hampshire’s Hospitals, where possible. You’ll respond to referrals from ambulance crews through the social work line which may include someone who has had a fall at home, needs urgent support and may require rapid community and social care services.
As a qualified, experienced Social Worker, you’ll work both independently and as a member of a multi-disciplinary team working closely with our SCAS colleagues. You’ll use a strengths-based approach to make confident decisions, even where there is uncertainty. Through effective communication with the ambulance crews, individuals and families, together with sound risk assessment and problem-solving skills, you’ll seek alternative solutions that result in non-conveyance to hospital. Your strong IT skills will support your work across both Hampshire County Council and SCAS IT systems.

You may be required to visit other locations within Hampshire and therefore need to have the ability to travel, as necessary.
