Specialist School Nurse - Inclusion

Main purpose of the job: To support children, young people and families who are Electively Home Educated to support CYP to engage with an educational provision through referral and escalation to Supporting Families/ Children Missing from education (CME) team. To be the allocated named School Nurse for three Alternative Provisions (AP) within Southend-On-Sea. To work in partnership with a wide range of professionals including but not limited to, GPs, Health Visiting, School Nursing, Community dental, Community paediatricians, Mental health services, Open Road, Supporting Families, Social Care, and the Youth offending team to ensure that children and young peoples health needs are supported, and safeguarding is maintained. To engage with safeguarding processes to ensure concerns are escalated through appropriate channels and a suitable plan of support can be developed to meet the need of the Child/Young person. This includes adhering to the local SET and national safeguarding policies and procedures, accessing mandatorysafeguarding supervision, liaising with the locality safeguarding team and social care, writing safeguarding reports and attending Child Protection conference, core groups and Child in Need meetings. Completion of safeguarding health needs assessments to support in the identification of health needs and inform a personalised plan of care/interventions in collaboration with the child, young person, and their parent/carer. To establish and maintain relationships with voluntary agencies to support the understanding of health needs, increase accessibility to services and address inequalities. To support effective service responses and pathways where there are multi-agency service interfaces required. To maintain an excellent standard of records for the service. It is recognised that children and young people within this client group often present with complex and/or enduring needs which require a multidisciplinary and flexible approach in the aim of recognising and addressing barriers to services and reducing health inequalities. Due to these factors, children and young people in this client group may be at increased vulnerability. To promote and upskill the Southend Children, Young People & Family Public Health service (0-19 team) and other agencies regarding this client group. Key responsibilities: To provide a proactive and responsive service, working flexibly to meet the needs of children and young people in this client group utilising a variety of communicative tools such as face to face appointments at alternative education provisions, home visiting, community-based appointments at family centres, telephone contact, video calls and Chat Health text messaging service. To be responsible for setting up, establishing, and maintaining School Nurse led drop-in clinics for young people at allocated alternative provisions. To be responsible for drop-in clinic with support from 5-19 skill mix for parents/carers for Young People who attend alternative provisions or who are Electively Home Educated. To complete and review risk assessments to identify safest means of School Nursing service delivery. To utilise problem-solving skills to address difficult to reach clients and families and encourage engagement with services. To develop evidence-based health promotion and education sessions for children and young people to support increased knowledge and understanding of effective management of health needs. To arrange meetings between parents and education to support the development of health needs care plans and effective implementation. To work alongside the Inclusion Team at the local authority and review any health needs of young People being discussed at the Inclusion panel. To work in partnership with a wide range of professionals including but not limited to GP, Health Visiting, Community Dental Service, Community Paediatrics, Mental Health services, Stop Smoking Service, Open Road, Social Care and Youth Offending Team to ensure that children and young peoples health needs are supported, and safeguarding is maintained. To supporting children missing from education to engage with an educational provision through referral and escalation to the Supporting Families/ Children Missing from Education (CME) team.To complete onward referrals to relevant services to support children and young people being able to access education. To have an in-depth understanding the legislation that underpins safeguarding practices and childrens rights to education and support. To engage with safeguarding processes to ensure concerns are escalated through appropriate channels and a suitable plan of support can be developed to meet the need of the child/young person. This includes adhering to local Southend, Essex and Thurrock (SET) and national safeguarding policies and procedures, accessing mandatory safeguarding supervision, liaising with the locality safeguarding team and social care, writing safeguarding reports and attending Child Protection Conferences, Core Group, Child in Need and Strategy Meetings. Completion of safeguarding Health Needs Assessments to support in the identification of health needs andinform a personalised plan of care / intervention in collaborationwith the client and their parent/carer. Attendance at specialist multidisciplinary meetings with a Child Sexual Exploitation (CSE) focus to increase understanding of CSE and Criminal Exploitation (CE) risk within the locality and how best to support vulnerable children and young people most at risk. To provide leadership within the team and to client group, acting as a positive role model by demonstrating excellent standards of working practice and communication. Accessing specialist training sessions to develop and promote skills needed to support children and young peoples varied and/or complex needs on subjects such as Gangs and County Lines, CE, CSE, Risk Taking Behaviours, Sexual Health, Smoking Cessation, Drugs and Alcohol, Mental Health, Risk Assessment and Communication. Evaluating and auditing service provision to ensure it is client accessible and effective. To support the Team Leader for the School Nursing service by providing advanced clinical practice and team oversight including knowledge and experience. This includes supporting on safeguarding practice, caseload oversight, statutory 5-19 programmes, training, audit, and interagency work as required. Support School Nursing Duty providing cover as appropriate. Promoting awareness and utilising school nursing referral system. To provide advanced public health clinical expertise to the public health team and school nursing team offering specialist advice and guidance for care of the 519-year-old children and young people at risk of exclusion and EOTAS. To support the development, implementation, and evaluation of policies/evidence-based guidelines for the service. This includes the quality assurance framework for the service. Support management oversight for clinical effectiveness of the Healthy Child Programme using evidence-based practice and audit. The post holder will adapt and be responsive whilst supporting development of the school nursing role, creating, and maintaining an environment in which excellence in service delivery will flourish. Support the team on safeguarding agendas that impact on the 5-19 services including Safeguarding Partnership subgroups (CSE/CE/Missing), primary care interfaces, multi-agency groups, and engagement with the universal services. Support the School Nursing Team Leader for the School Nursing service with management of the service and the skill mix team as appropriate. Clinical To work with children, young people, and their families to deliver the Healthy Child Programme 5-19 whilst supporting statutory safeguarding delivery. To establish and maintain relationships with other statutory and voluntary agencies e.g., primary care, 0-5 universal services, safeguarding teams, specialist/targeted childrens services, Specialist School nursing, Supporting Families, social services, and education. Provide a supervisor/preceptorship role to School Nurses and Health Visitors, identifying ongoing developmental needs. Support effective service responses and pathways where there are multi-agency service interfaces required. This includes but is not exhaustive of dealing effectively with child protection processes, domestic violence incidents, A&E notifications, and mental health notifications.
