Full time Post 37.5 hours per week)

GBP26-GBP28 per hour Umbrella Rates

About the Service:

Initial assessment of swallowing and communication difficulties following acute stroke. Long-term rehabilitation of stroke patients as part of the core multidisciplinary stroke rehabilitation team.

Information on the Role:

• To provide stroke survivors with aphasia, their families and carers with Speech and Language Therapy support which enables them to adapt to life after stroke.
• To be provided a structured programme of Speech and Language Therapy on a group and 1-2-1 basis, by offering a person centred approach, which is goal oriented and is a casework based service. Seeking to achieve desired communication outcomes.
• Completing assessments and reviews of the client s needs, referring on to other services where appropriate ensuring information about local services, referral options and sources of support is relevant and appropriate to the service user and is kept up to date
• To develop and maintain effective working relationships with Speech and Language Therapy and stroke teams across acute and community Settings including Social Care to ensure the smooth transition of service users through the system, ensuring the service user s requirements are met consistently
• To provide appropriate feedback as required, per contractual and service specification and with responsibility to produce accurate data and reports to fulfil required routine monitoring information
• Ensure Speech and Language Therapy team members contribute to the development and delivery service users stroke recovery plans

Essential Requirements:

• Must have stroke experience and preferably general medicine experience also
• Primary Degree in Speech and Language Therapy
• Good IT skills
• Excellent written and communication skills
• Enhanced DBS on the Update Service
