We are looking to recruit for a motivated Band 5 nurse to join our friendly team within the Transfer Unit
You will need to be able to problem solve, work well under pressure and have good attention to detail in order to ensure that each patient is discharged safely.
The successful candidate will also need to be passionate about caring for patients and ensuring the last memory of their hospital journey is positive.
The ideal candidate will have experience of MDT working and will have excellent organisational and communication skills.
The successful candidate will be compassionate, caring and committed to provide a high standard of patient care.
Working as a registered nurse within our team you will have the opportunity to work closely with the Patient Flow Team, The Discharge Team and the wards within the hospital
As a Band 5 staff Nurse you will be expected to communicate effectively with all the wards and departments liaising closely with the Patient Flow Team, patients' families and carers Team to ensure the smooth running of discharges within the hospital
The successful candidate will need to be confident in their clinical abilities as you will be expected to be working automatously.
The successful candidate will provide holistic care of all patients who pass through the Transfer Unit
You may be required to refer patients to District Nurses
You will be required to administer medications
Liaise closely with the Transport Booking Clerk to ensure transport home has been booked
The successful candidate will need to be available to work early or late shifts over 4 days a week Monday to Saturday

The post holder will be responsible for effectively delivering skilled nursing care to patients using a systemic approach of assessing planning, implementing and evaluating to ensure a high standard of care. The post holder will assist in the admission and discharge processes for patients in line with local procedures.
They will provide support with the effective patient admission pathways and effective discharge planning using their clinical skills to agree admissions and manage/report any impact on shifts/ workload and staffing levels.
They will proactively engage with patients, relatives and carers to seek their views, gain agreement and keep them informed about their treatment and care. They will demonstrate compassion by recognising and responding to individual needs in a warm, thoughtful, sensitive and helpful way.
Maintain registration, and nursing competencies, and comply with NMC code of Professional Conduct and related documents.
To be flexible to work within other areas or departments to meet the demands of the trust workforce model and changing service needs.
If successful, this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary. In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
