Job Title: Support Worker
Job Purpose:
- Compile and implement personal care plans providing practical, physical, social and emotional care and support to service users at Mid-Lin Day Care.
- Arrange and undertake formal reviews in relation to service provision and current legislation.
- Assist/support Service users with all aspects of care maintaining privacy and dignity at all times.
- Assist to create a supportive atmosphere in which service users can achieve maximum independence.
- Adhere to Mid-Lin Day Care’s policies and procedures at all times in carrying out duties.
- Liaise/consult and work with carers, health professionals and other agencies involved in individual’s care working in a holistic approach to service provision.
Main Activities
Provide practical, physical, social and emotional care/support in relation to the identified needs arising from the individual’s personal plan.
You will be required to be sensitive and responsive to the race, culture, religion, age, disability, gender and sexuality of the client who is receiving care and their representatives.
To plan, arrange deliver and encourage service user participation in the activities program. At Mid-Lin a wide range of activities is offered and delivered to interest and stimulate the physical, emotional and mental well-being of all who use the service at Mid-Lin.
Guide, encourage and instruct volunteers/placements to deliver all aspects of the service provision. The activities are a tool to motivate and guide the volunteers/placements in the activity programme, under the direction of the manager.
Specific Duties
Assist and deliver care according to the individual service user’s specific identified needs and the National Care Standards.
Your duties will include:
- Communicate effectively with service users, family members, other relevant agencies and the Mid-Lin staff & volunteer team.
- Have an in-depth working knowledge of the individual needs of all assigned service users.
- Information sharing with team members, family members and other relevant bodies as appropriate in a timely manner.
- Keep accurate, factual up to date records in accordance with the organisations policies and procedures.
- Observe and follow the Mid-Lin Day Care Health and Safety policy and other relevant policies, procedures and legislation.
- Support and encourage service users to participate in a range of activities
- Support service users to exercise choice as appropriate to their individual abilities

Personal Care
Your duties will include:
- Assist service user to dress and undress
- Assist in the management of continence
- Assist in moving and handling using aids.
- Deliver all aspects of individual personal care with privacy, respect and maintain dignity at all times.
- Support service user to maintain their level of independence
- When required, assist service user with eating and drinking
Other Duties
- Appropriate other duties which may be part of a service user’s care plan.
- Assist with light household duties which may include: Laundry, dusting, vacuuming, tidying, helping with dishes, shopping
- Assist in making transport arrangements for service users
- At times, escort service users to and from home on Mid-Lin transport
- Provide support to other staff members and volunteers at Mid-Lin
