The role
The Community Wellbeing Directorate within Herefordshire Council are developing adult social care services in keeping with the responsibilities under the Care Act 2014. We are looking for a full-time Urgent Care Coordinator, who will bring excellent communication skills, knowledge of operational processes combined with a pro-active and can-do attitude. You will be expected to have experience of working in Adult Social Care or a National Health Service organisation and be familiar with the process relating to discharge from hospital to home. This particular role is focused on coordinating the Discharge to Assess and the transfer of care process, working within the Integrated Discharge Team. The role is crucial to ensure that adults are assessed in a timely manner and next steps are identified. This role involves multi-agency working with a wide range of partners including health professionals, Adult Social Care Teams, Commissioning and other external agencies including Home First. The post holder will need to be an expert with excel spreadsheets and compiling reports based on data generated from the process. The work is fast paced and you will need to manage different demands within tight timescales. It is essential to the role that you are able to challenge and escalate any issues along the pathway and are accountable for the co-ordination of organisations to ensure the flow from hospital to home. You will receive regular supervision and work in a supportive environment with other social care staff. Herefordshire is a rural county, travelling may be required with an expectation to attend Multi-Disciplinary Team Meetings weekly within Herefordshire.
There is an expectation that the post holder will support the wider system of integrated working and have a knowledge of Discharge to Assess and Adult Social Care pathways.
