Job description

This post is funded through the Health & Wellbeing Grant through NHS Cheshire & Merseyside and Cheshire West & Chester Council.

We are looking for someone to take forward our successful project ‘Bridging the Gap’ which was a new project within the organisation from January 2022. This is extension funding for an additional 2 year period until 31.12.25.

The work focuses on reducing health inequalities and access to primary care for Black, Asian and Minority Ethnic (BAME) communities in Cheshire West. The project has three key aspects:

1.     Health promotion within BAME communities

2.     One to one support for individuals

3.      Healthcare support to health agencies

Duties & Responsibilities

1.     Develop key materials to market and promote the project

2.     Network, collaborate and maintain relationships with local VCFS organisations, public sector and healthcare providers to promote the project and its objectives.

3.     Engage with BAME communities across Cheshire West to understand their health care needs and enable them to access health services

4.     Pro-actively support individuals from BAME backgrounds to access healthcare and support them to navigate UK systems, including advocating on their behalf with healthcare providers and others.

5.     Play an active role in relevant health related partnerships and attend meetings of the same.

6.     Provide support and advice to both statutory and VCFS healthcare providers around issues that affect BAME communities.

7.     Organise events around key health issues affecting BAME communities to raise awareness and reduce inequalities.

8.     Seek and maintain links with individuals who can provide translation support.

Administration

1.     Undertake continuing monitoring and evaluation in line with the funding agreement.

2.     Collate data around individuals barriers and opinions of the health care system.

3.     To complete relevant monitoring reports for the funder.

4.     To provide written reports and statistical information on a regular basis.

5.     To maintain information confidentially and in line with good ethical principles and GDPR.

Other

1.     Contribute to the organisation’s monthly newsletter and social media posts.

2.     Promote the organisation and its wider services.

3.     To undertake other duties appropriate to the post as instructed by the Director.
