SalaryWe are currently looking for 3 Social Workers with significant experience of safeguarding adult work and complex case management.
The Social Workers should have experience of working in Hospital Discharge setting; working in multi-disciplinary setting where everyone work to achieve safe discharge and the best outcomes for patients. Social workers should have experience of working with people and taking into consideration their views and wishes and balancing family dynamics at the point of crisis. As Social Workers they should work to advocate for patients and their families and making sure that they have been signposted to appropriate services to support their discharge from the hospital. Social workers should be confident in undertaking assessments of needs, setting up support plans and identifying appropriate intervention. They should have good understanding of the Strengths based approach and its application in practice. Social workers should be able to undertake capacity assessment and best interest and ensuring those who lack capacity are referred to advocacy.
