Role: Adult Social Worker- Hospital Team.

Salary: £33,820 - £36,296 - Pay award pending

Are you a qualified and registered Social Worker with at least one year of post qualifying experience? Do you have a passion for supporting adults with complex needs and their carers? Do you want to join a dynamic and supportive team that works closely with hospital staff and community partners?

If so, we have an exciting opportunity for you to join the Hospital Team at Leeds. You will provide a high quality and comprehensive social work service to adults who are being discharged from hospital or who are at risk of hospital admission. You will work under the general direction of a team manager and with support from senior Social Workers.

Your Duties and Responsibilities will include:

Carrying a caseload and undertaking assessment of service users and carer needs using a person centred approach in accordance with agreed statutory and directorate procedures.
Devising and implementing service and support plans that correspond to the assessed needs of service users, their families and carers in line with procedures.
Analysing information to support the ongoing needs of service users, carers and their families.
Monitoring and contributing to the review of plans relating to the assessed needs.
Ability to plan, prioritise and time manage own workload.
Able to exercise individual judgement, assess risk and consult where and when appropriate.
Requirements:

Registered with SWE (Social Work England)
Appropriate Social Work professional qualification (DipSW), CQSW, BA, BSc, MA)
Enhanced DBS.
At least 12 months experience.
