About the Team
Tandridge Social Care team are based at the local council offices in Oxted, in which we share our base with other services such as district nurses, housing, wellbeing and other council workers. We work with an array of adults with varying cognitive and physical impairments, as well as paid and informal carers, family members, and multi-disciplinary professionals. Our day-to-day role is to assess people's care and support needs and any informal carers under Care Act 2014. Through this assessment we determine eligibility and build a support plan of care that best works for an individual and their situation. We have a wealth of knowledge and varied experience within our team and are well known for our supportive culture that we hold for all our team members.
About the Role
As a Social Care Assistant at Surrey, you will often be the first point of contact between the public and the service, gathering initial information and determining the need for further assessment as well as signposting people to other services.
You will have frequent direct contact with the public on the phone as well as visiting them in their own home or care setting. We will support you to be in the best possible position to assess and review the Social Care needs of our service users and their carers. With the mentoring and support from your experienced colleagues, we will enable you to reach your full potential.
You will work in partnership with individuals, their carers, professional staff in other agencies, service providers and Support Brokers to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control. This is an exciting opportunity to work with people to maximise their potential for independence.
Within the role, you may work with any combination, or all, of the following care groups: older people, people with physical, sensory, cognitive impairments, people with a learning disability and people with mental health problems.
