Qualifications, Experience & Training required NMC/regulatory body registered with up to date mandatory training Advanced Nurse Practitioner/ACP Qualification with Masters Degree in Advanced Clinical Practice (equivalent or higher) Clinical skills qualification at degree level, Experience of Primary Care Evidence of completion of study in minor illness, Independent prescriber, Evidence of completion of study and/or experience in Paediatric and Geriatric care Ability to autonomously carry out consultations with patients - taking a comprehensive patient history; physical examinations and use of your expert knowledge and clinical judgement to identify potential diagnosis. Decide on and carry out treatment, including the prescribing of medicine or referring to primary or secondary care specialist services within local agreed pathways and protocols as appropriate, Provide clinical advice and mentorship to junior Clinical staff. Experience of working autonomously and being flexible with daily demands. Be experienced with demonstrating critical thinking within a safe clinical decision-making process. Ability to carry out clinical assessment, diagnosis, treatment and evaluation of care. Ability to carry out home visits to house bound patients and those in care homes. Experience of Chronic disease management Understanding of required target driven activity such as QOF and enhanced services as well as seasonal projects Understanding of the GP contract and DES requirements Experience desired Knowledge of EMIS web and Primary Care systems Knowledge of managing Care Home ward rounds or experience of Enhanced Health in Care Homes DES Care planning and managing EOL patients Chronic disease specialties with the ability to take the lead within the team i.e. Diabetes Ability to lead and support junior Clinical staff in development of Chronic Disease management to ensure a full and robust skill mix across the Practice for the care of the patients.
