To lead on the running of a support group for transitional patients and their family/carers. To actively involve service users in providing feedback of their experiences of the current service and suggestions for improvements through facilitation of support groups and transition workshops also roadshow events. To maintain contemporaneous and accurate treatment records, submitting relevant statistics, reports and activity data as required to a high standard. To take a lead in developing the quality and availability of transition related information for young people and their family units. To ensure the Trust has appropriate policies and processes for Transition to Adult Services are effective and evidence-based care in conjunction with other members of the multi-professional team To provide accurate and timely information to inform Trust/National databases as required To work within local and national frameworks to implement service changes To lead in the provision of high-quality teaching and training in the area of Transition to adult care/services in collaboration with national and local guidance and input from experts by experience To firmly embed the early help agenda within the Transitions service by identifying risks or vulnerabilities that could lead to harm or abuse without intervention Promote equality, diversity, and rights, and treat others with dignity and respect ensuring services are developed, managed, and delivered to meet the specific needs. Promoting health and welling by tackling inequalities. Representing and reporting to Trust sub-board committees, such as Children and Young People Meetings internal .
